Your First Visit

On your first visit to our office, you will be welcomed as a member of our family and our receptionist will greet you with a warm smile. You will be asked to fill out a New Patient Entry Form and a Pain Diagram which will help us gain some information about yourself and your condition. Please take your time to fill them out completely. The more information you are able to provide, the better we will be able to help you. All forms will remain strictly confidential (see our privacy policy for more information).

The doctor will ask you directed questions to assess your condition and how it is affecting you, and then will proceed with an examination. Nothing will be done in our office without your consent or level of comfort. A spouse or family member is always welcome to attend.

You will receive a thorough physical exam which will include standard orthopedic, neurological, postural, and other examinations as required. After the initial examination, the doctor may order additional testing to be performed in order to firmly diagnose your condition. No additional testing will be performed in our office without a full explanation of the procedure, your options, and your consent.

Additional Testing that may be ordered may include X-ray, CT, MRI, EKG, or Laboratory Testing. If you have recently had testing done, please bring in copies of the films, reports, and laboratory results to your first appointment along with a list of your current medications (including herbal and over-the-counter).

Once all the information and examinations have been performed, the doctor will give you a report of findings and tell you his recommendations for treatment. Your treatment may include chiropractic, massage, physical therapy, rehabilitative exercises, nutritional advice, or any combination of these.

Our office participates in many insurance programs. We also accept auto accident liens and workers’ compensation cases. To find out if your insurance covers chiropractic care, you can call The Chicago Body Works directly.

For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged.

If you have any questions regarding your care at our office, please feel free to speak with any office staff members or the doctor. We want you to feel comfortable and confident with your treatment here, along with knowing that your health care is our first priority.
